Collection Procedures

Each patient will be provided an estimate of their financial responsibility on or before the date of service. Payment of estimated financial responsibility is expected prior to surgery. Once the claim is processed by the insurance company, prompt payment of any remaining patient balance is expected. Estimates are provided based upon anticipated services. Final charges will be based upon actual services provided to the patient.

Uninsured Patients

The facility may offer a discount to patients who do not have eligible health insurance coverage or is uninsured. The discount is a percentage reduction of charges and is subject to change.

Provider Information

During your visit at this facility, services may be provided by other health care providers who may separately bill the insurance company and/or patient. The other health care providers may or may not participate with the same health insurance companies as the facility. Patients may request a personalized estimate of charges and estimated financial responsibility from the facility and any other health care provider that may provide services during the visit. The patient is encouraged to contact the health care providers and/or their insurance company to verify participation status.
